Insurance Information
MBK Nutrition is currently an “in-network” participating provider for the following plans:

Please Note: Even though MBK Nutrition is considered “in-network” with your current insurance plan, please call the member services # listed on the back of your card to inquire what coverage, if any, you have for “nutrition counseling” specifically.
Some plans have nutrition coverage, and others do not. You can let the representative know that it's an office visit, I am an “in-network Registered Dietitian” and that I use these CPT codes: 97802 and 97803 for Medical Nutrition Therapy. If you have coverage, find out what your copay or coinsurance is, how many visits will be covered, do you need a referral, and is nutrition counseling diagnosis specific. (For example, some plans only cover nutrition counseling for diabetes.) Some plans do have preventative benefits built in, so I recommend asking if this code (Z71.3) is a covered benefit and how many visits are covered under that code. If you have an eating disorder diagnosis from your therapist or physician, please ask for that diagnosis code and ask your insurer if that is a covered condition for nutrition counseling. Also, ask for a reference # for the call, so you have a record of the conversation in case there is a problem in coverage after the claim is submitted. It is your responsibility to obtain this information before the initial appointment. If your insurance plan denies the visit, you will be responsible for the appointment cost as explained in the MBK Nutrition Office Policy.
Registration and Office Policy Forms
All registration information, including the Nutrition Intake, Office Policy, Release of Information, & Privacy Practice forms, are completed electronically through a secure client portal. Once we schedule an appointment, I will send you an invitation to join the portal and all your forms will be stored there.
*Please note: there is a 48 hour cancellation policy for all appointments, as stated in the Office Policy Form. A credit card is required on file to secure appointments.
